What is Perinatal Care Coordination (PCC)?

The Perinatal Care Coordination model is designed to increase access and enrollment into available perinatal resources during the prenatal and postpartum period for individuals and their families. We currently have staff embedded in local OBGYN clinics providing onsite support as well as onsite at our local public health departments.

Perinatal Care Coordinators support pregnant individuals access medical care and resources during pregnancy and after their baby is born.  There are no income or insurance requirements to meet with a Perinatal Care Coordinator to learn about resources and get support.

Perinatal Care Coordinators partner with families to provide:

  • Information and referrals to Perinatal (Prenatal and Postpartum) resources
  • Medical Insurance Application Assistance
  • Nutrition Resources, including enrollment onto the WIC program
  • Prenatal, Medical, Behavioral Health & Dental Care Coordination
  • Arranging Medical Transportation
  • Information on Childbirth Education and Lactation support in the community
  • Information and referrals for personalized nurse support services and other family support services in the community

Eligibility

  • Any Pregnant or Postpartum individual.  (some program enrollments are dependent on income)

Oregon MothersCare

About Oregon MothersCare (OMC)

Oregon MothersCare (OMC) seeks to improve access to prenatal care for all people in Oregon. OMC started with five sites in January of 2000 and grew to twenty-five sites by 2018. OMC services are provided at Access Sites, typically local clinics or public health agencies. OMC Access Sites help connect pregnant people to the services they need for a healthy pregnancy. OMC sites provide these services to all clients free of charge.

What to Expect at an Oregon MothersCare Visit

  • A no cost pregnancy test, if you need one
  • Help applying for the Oregon Health Plan (call ahead so you know what to bring)
  • Learn how to make a prenatal appointment and dental appointment early in pregnancy
  • A chance to ask questions
  • Learn about things you can do to stay healthy for you and your baby
  • Connections to programs like nutrition assistance from WIC or other helpful resources
  • Excellent staff who work with pregnant women every day
