The eligibility process begins with a call to the eligibility team. You will be guided through the application and intake process.
Deschutes County will then gather records and review them to determine if the requirements are met under the Oregon Administrative Rules. Phone 541-322-7554 and request an intake for services. The Intellectual/Developmental Disability Program provides information about resources, connects clients and families with existing programs and facilitates the development of needed services. Service Coordinators advocate for clients and provide assistance as they and their families transition through the stages of development.
Community First Choice State Plan (K-Plan) allows Oregon to provide home and community-based services and supports while receiving a 6% increase in federal medical assistance funds from the federal government for those services.
The services provided benefit Oregonians who want to stay in their home and remain independent, healthy, and safe. At the same time, it saves both the state and federal government money because we are providing more extensive supports and services in a less restrictive environment meaning in lieu of more expensive institutional-type care.
When adults and children with I/DD are no longer able to live or be cared for in their own or family homes, foster care is an option.
