If Deschutes County Health Services has contacted you regarding your status as a close contact to someone who has tested positive for COVID–19, or if you have tested positive for COVID-19, you may request an employer letter.

This is for individuals only. Employers may not request a letter for their employees.

Please specify only personal email and home addresses. We will not send letters to employer addresses or email accounts. Deschutes County does not share personal information. Email address will only be used to provide you a letter, and to possibly contact you for any further information that may be needed. Most items below are required entries.
