Transfer Prescriptions

TRANSFERRING A PRESCRIPTION IS SIMPLE; JUST CALL OR VISIT DOC PHARMACY

 

If you have a prescription on file at another pharmacy, we need the following information:

  1. Patient name
  2. Pharmacy name
  3. Pharmacy phone number
  4. Prescription number
  5. Prescription name

If you have a pill bottle from an existing prescription, please bring it to DOC Pharmacy. We’ll take it from there!

If you have discarded the bottle or don’t have this information, we’ll call your doctor for a new prescription. This includes your mail order 90-day prescriptions.

 

Anyone enrolled in the County Employee Benefits Plan, including employees (and their dependents) of Deschutes county or COIC, retirees, and people enrolled in COBRA coverage may use the Deschutes Onsite Clinic (DOC)DOC Pharmacy, and Well Connect. The clinic practitioners will see adults and children ages two years and older.


DOC services are not available to the general public. Please visit the Deschutes County Health Services department for services offered to the public.
