Group and Voluntary Life Conversion

Find out how you can convert or port your life insurance coverage(s) into individual policies when leaving employment. 

Life Insurance

As a participant in the Deschutes County group term life insurance plan, your life insurance coverage ends at the end of the month in which you are no longer an active employee. You may be eligible to convert your Deschutes County group, voluntary life insurance, or voluntary Accidental Death & Dismemberment (AD&D) coverages,  or port your current voluntary life insurance to an individual policy. The advantage to converting or porting is that there is no requirement for you to complete a medical examination to qualify for coverage; however the cost of converting or porting is much higher than Deschutes County employee premiums.

Your group life insurance benefit is one times your annual salary, rounded to the nearest thousand dollars, up to a maximum of $100,000. This does not include overtime pay, but includes most additional pays.  

Your voluntary life coverage amounts can be found in Employee Self Service (ESS)/Benefits.  Click on the Voluntary Life benefit to show the drop down with coverage and current premium amounts.

If you decide you'd like to convert/port any or all of your policies, please contact Human Resources - Benefits at benefits@deschutes.org or call (541) 317-3154.  A completed Employer Notice will be sent to you, which you will send along with your completed application to the appropriate insurance company.  The application to convert/port must be received by the insurance company within 31 days of the date your active employee coverage ends.  

Long Term Disability (LTD)

As a participant in the Deschutes County Long Term Disability (LTD) plan, your LTD insurance coverage ends on your last day of employment in an eligible position.

Accidental Death & Dismemberment (AD&D)

As a participant in the Deschutes County group Accidental Death & Dismemberment (AD&D) plan, your group AD&D coverage ends at the end of the month in which you are no longer an active employee.  If you are enrolled in voluntary AD&D, see information on conversion above under Life Insurance.
