Living Well with Chronic Conditions

The Living Well with Chronic Conditions program is a six-week workshop that teaches practical skills for living a healthy life. Chronic conditions include diabetes, arthritis, HIV/AIDS, high blood pressure, depression, heart disease, chronic pain, anxiety, multiple sclerosis, fibromyalgia and others. If you or someone you care for has one of these conditions, the Living Well Workshop can help you take charge of your life.

A Proven Program

The Living Well with Chronic Conditions program, also known as the Chronic Disease Self-Management Program (CDSMP), was developed and tested by Stanford University. It was designed to complement and enhance medical treatment and disease management. Research has shown that participants are able to manage their symptoms better and communicate more easily with their doctors and loved ones. People who complete the program feel better, are less limited by their illness, and may spend less time at the doctor or in the hospital.

The Oregon Health Authority (OHA) supports Living Well workshops in Oregon by providing opportunities for leader training and networking. Visit their website to learn more.

Workshops

For people with ongoing health conditions and those who care for them

Join a 2 and ½-hour Living Well Workshop, held each week for six weeks. Classes are fun and interactive. Participants share their successes and build a common source of support. The workshop builds confidence around managing health, staying active and enjoying life.

Learn from trained volunteer leaders with health conditions themselves about how to:

  • manage symptoms and medication
  • work with your health care team
  • set weekly goals
  • problem-solve effectively
  • improve communication
  • relax and handle difficult emotions
  • eat well and exercise safely

Set your own goals and make a step-by-step plan to improve your health—and your life. Each participant in the workshop receives a copy of the companion book, Living a Healthy Life With Chronic Conditions.

Living Well with Diabetes

This class was designed to help people living with diabetes or their loved ones,  gain the confidence and motivation to better manage symptoms and the day-to-day challenges of living with diabetes.   By developing self-management skills, one can be empowered to actively achieve better health and well-being.

Workshop Details
You will learn techniques and tools to deal with the symptoms of diabetes such as:

  • Healthy eating and menu planning
  • Preventing and minimizing the complications of diabetes
  • Strategies for including exercise into your daily routine to maintain or improve strength, flexibility, and endurance
  • Communicating effectively with family, friends, and health professionals
  • Monitoring blood sugar
  • Managing sick days
  • Problem solving, goal planning and stress management

The six-week workshop is held once a week for 2 1/2 hours each. The workshop is facilitated by a certified peer leaders.. Each participant in the workshop receives a copy of the companion book, Living a Healthy Life with Chronic Conditions.

Tomando Control de su Salud

If you or someone you love, struggle with health conditions such as diabetes, heart disease, arthritis, cancer, or lung problems, help is on the way. 

The Tomando Control de su Salud (also known as Living Well with Chronic Conditions) program was developed and tested at Stanford University's School of Medicine.  The program is a workshop that is given two and a half hours, once a week for six weeks in community settings such as churches, health departments, libraries, and senior centers.  Spanish-speaking people with different chronic health problems attend together. All workshops are done in Spanish by two trained leaders without interpreters.  Patient participants must speak Spanish but it is not necessary that they read Spanish.

Through the series of classes, participants learn how they can feel better and manage their condition more effectively.  An example of topics covered in the classes includes:

  • Techniques to deal with problems such as frustration, pain and isolation
  • Appropriate exercise for maintaining and improving strength, flexibility and endurance
  • Understanding new treatments and appropriate use of medications
  • Healthy eating
  • Communicating effectively with family, friends, and health professionals

Each participant in the workshop receives a copy of the companion book, Tomando Control de su Salud: Una guía para el manejo de las infermedades del corazón, diabetes, asma, bronquitis, enfisema y otros problemas crónicos, an audio relaxation tape, Casete de Relajación, and an audio exercise tape with booklet, ¡Hagamos ejercicio! All materials are in Spanish.
