Deschutes County Birth Registration

How to Order a Birth Certificate

Birth Certificates are filed at Deschutes County Health Department for hospital and home births occurring in Deschutes County.

Birth Certificates can be ordered by mail or in person from the Vital Records Department at Deschutes County Health Department for births occurring within the last six months. After the child turns 6 months of age, birth certificates must be ordered from the
 Oregon Vital Records State office.

If your child is under 6 months of age;
You may come into the Deschutes County Health Department office and fill out a Birth Certificate Request Form or download a copy from this website.  You will need the free Adobe Acrobat Reader in order to view this form.  You can download a free copy of this program by clicking on this link.  http://www.adobe.com/products/acrobat/readstep2.html
