What to Bring:
    *
 Identification for yourself and each child you want to add to the program
    * Verification of Income
    * Verification of residency (Bills or other mail, Oregon driver's license, etc)
   

    Your Oregon Health Plan (OHP) card can be used in place of all 3 of the above
