Your Personal Health Assessment (PHA) will provide insight on your current health and empower you to lead a healthier lifestyle, as well as provide you savings on your health insurance premium.

 

NEW for 2018:


1) Only TWO steps instead of three.

2) ANY labs drawn since January 1st, 2018 that include a lipid panel and fasting blood glucose will count towards you PHA.

3) We will also be making more trips to select department locations for initial appointments; See Wellness Calendar for off-site visits dates and times.

"I had known that I had to make some health changes. I felt awful, I had gained weight and I was using every bit of what energy I had for work. I didn't do the PHA initially because I didn't like my health condition; it is funny how you avoid knowing the numbers when you most need that information.

When I answered the health questions this past year for the PHA, I answered that I was planning to make a change, and I did. I committed to myself to drink 64 oz of water, walk 10,000 steps a day, get 8 hours sleep, not eat after 6pm, and lower my daily calorie intake by 30%. I held myself to that commitment, and gradually started increasing my exercise level.

I went in and had a blood screen done and my cholesterol and all my stats have improved. I feel so much better, and now able to do activities beyond work!"

Anyone enrolled in the County Employee Benefits Plan, including employees (and their dependents) of Deschutes county or COIC, retirees, and people enrolled in COBRA coverage may use the Deschutes Onsite Clinic (DOC)DOC Pharmacy, and Well Connect. The clinic practitioners will see adults and children ages two years and older.


DOC services are not available to the general public. Please visit the Deschutes County Health Services department for services offered to the public.
