Death Certificates can be ordered by eligible persons* at Deschutes County Health Services for all deaths occurring in Deschutes County within the past six (6) months.
After six months have passed since a death, the certificates must be ordered from the Oregon Vital Statistics office at (971) 673-1190 or at www.vitalchek.com
Requests for death records may be made in person or via mail and are normally mailed out within a week, assuming no problems are found. Customers ordering in person may request to pick up a certificate the following day (after 2:30 p.m.), also assuming no problems are found.
The fee for a certified copy is $20 for the first copy and $15 for each additional copy ordered at the same time. Please include a copy of current photo ID with each order.

* Access to death records filed in the State Vital Records office is restricted for 50 years after the date of the event to family members and their legal representatives, persons or organizations with a personal or property right, government agencies and persons licensed in Oregon under ORS 703.430. When ordering a death record, you must state your relationship to the registrant, or your reason for needing the record. If you are a legal representative of a family member, include whom you represent and how they are related to the person named on the record. If you have a personal or property right, explain why you need the record. If you are not eligible to order the record, please include a permission note with the notarized signature of an eligible person.
