Birth Certificates

Birth Certificates may be ordered from Deschutes County Health Services for all births occurring in Deschutes County within the past six (6) months.

After 6 months have passed since a birth, the certificates must be ordered from the Oregon Vital Statistics office at (971) 673-1190 or you may order at www.vitalchek.com

Requests for birth certificates from Deschutes County may be made in person or via mail and are normally processed within a week, assuming no problems are found.  Customers can request to pick up a certificate in person the following day (after 2:30 p.m.), also assuming no problems are found. 

The fee for a certified copy is $20 for the first copy and $15 for each additional copy ordered at the same time.  Please include a copy of current photo ID with each order.

Order forms are given to new parents at the time of birth (at the hospital or birth facility). Otherwise, forms may also be obtained from the Deschutes County Health Services office and are available online here.
