Filing Forms

To place an argument or candidate's statement in the voters' pamphlet, download the appropriate form below. Form instructions provide information regarding filing deadlines, fees and other requirements. Completed forms must be submitted to the Deschutes County Clerk's Office by 5 pm on the filing deadline.

Filing forms are in PDF format and can be viewed using Adobe Acrobat© Reader, which may be downloaded for free if you do not already have it.

Filing forms for measures, explanatory statement, argument statement and the candidate's statement must be typed and the original filed with the Clerk's Office.

The explanatory statement, argument statement, and candidate's statement are also available in Microsoft Word format. If possible, please provide these forms electronically to the Clerk. The easiest way to do so, after completing and printing the original, is to save the document and email it to Nancy.Blankenship@deschutes.org.

The Notice of Measures is available electronically only in the pdf format. In addition to filing the original with the Clerk's Office, if possible, please provide the caption, question, and summary electronically to the Clerk. To do so, you can type the information in a word processing program and email it to Nancy.Blankenship@deschutes.org.

State forms and manuals may be accessed on the Secretary of State Publication and Forms website.
